The man was believed to be the super of the building, but it is not known how far he might have fallen in the 12-story building.
A second unidentified man was rushed from the building to Bellevue Hospital with trauma, but authorities offered no details of his condition or how he was hurt.
The city's Department of Buildings website said the elevator shaft in the 99-year-old building was "open" and "unguarded" causing a dangerous condition.
The agency was investigating the accident Wednesday afternoon, a spokesman said.
